Usually the index maps to the member number in the volume set. This is only
because of the pseudo standard of naming the master member1 and then the
members starting at 2. BUT this is not always the case. VOLUTIL showset will
display a map of the volume name to index. Also one can get an exact
dump of the ldev to index via the fs_volume_list debug macro.
