We have had a failure on two mirrored disks. (A pair).
Replacing one of the faulty drives with a spare gave this error on boot:
LLIO Error - subsys: 213, proc num: -49, error_num: -28
for path 10/4/4.2.0.
ERROR - LLIO device configuration error: -11
status - subsys: #150 info: #-11
The old, faulty disk was then re-mounted, the other faulty disk was
replaced with the same new spare. Gave this:
LLIO Error - subsys: 213, proc num: -49, error_num: -28
for path 10/4/12.2.0.
ERROR - LLIO device configuration error: -11
status - subsys: #150 info: #-11
LLIO Error - subsys: 213, proc num: -49, error_num: -28
for path 10/4/4.

The problem was then solved by rebuilding the mirror with the rest of
the OK disks - plus a reload of the data. So, we're up and running, but
missing a disk-pair. When the 2 new spare disks arrive from HP, I fear
that we will be in the same situation again.
Searching in the archives, I found a similar situation, the cure was to
re-reboot. This has not been tested.
Other sources point to mismatch between device IDs in Sysgen and
IODFAULT.PUB.SYS.
The two old disks are: HP A3647-67001, the new disk, that gives the
error, is a HP A3647-67003. They are in a Jamaica enclosure, MPE is 6.5.
Any help will be much appreciated.
