MSDTCW Page Fault problem solved! MSIE 4.01 updates the Remote Procedure Call DLLs. Evidently, one of these had been "downgraded" to a previous version, so although MSIE 4.01 was still reported as being installed, not all of the components were up to date. Reinstalling MSIE 4.01brought all of the rpc*.dll files back up to date and MS PWS 4.0 now works fine! And its a very nice personal web server, too!
