Employees who have Healthsource HMO must complete an enrollment/change form
during annual enrollment to change to the John Deere or BlueCross/Blue
Shield health insurance plans.  The Prepaid Plan 1 and Reimbursement dental
plans will not be offered in 2000.  Employees who have these plans must
complete an enrollment/change form during annual enrollment to change to
the Prepaid Plan II or the new PDO plan  to continue dental coverage in 2000.

Annual enrollment changes must be made by November 15. Changes can be made
by completing the appropriate form(s) at Personnel Services, Room 206, 400
Palmetto Street, or in the University Center as follows:
