We did that, as well as FLOCK/FUNLOCK,  block factor = 1 and
also I tried FSETMODE 2 but it did not stop the problem occurring.

From my experience with this problem, I would say one
way to reproduce it, is to write a small program which appends a record
to each of 2 files, pauses for a random small number of seconds, and repeats
this forever. Then, you launch 40 or 50 copies of this to execute concurrently.
(eg., by using MPEX GOON option). Let the processes run for a
'sufficiently' long time, (after about 10000 records) then analyze the
pair of output files.
If I was still working on the problem, that would be my next step.
