One shouldn't forget CIA (Computer I/O Analyser) written by
Neville Silverman, and marketed by Facer (Sydney, Australia).
I've used it for years and find it difficult to imagine how
I could manage a system without it.  Its very gentle on resource
consumption, and provides all of the above, both online and
as a batch job providing graphs and reports (with a neat
facility for downloading daily summary statistics to a PC
and a VB interface for the graphs).
