Look in the server's error log whenever you experience "500" problems.  I
don't know where this is by default for the NCSA server.

I've experienced the "cannot fork" problem with Apache when I ran out of
transient disk space once.

You might consider switching to Apache/iX because NCSA 1.3 is extremely old
and probably has some security bugs.  Apache corrects the old bugs and adds
