I can answer questions a & b:
 
For MPE, we generally release a Powerpatch or an Express
approximately once a quarter.  A MPE release shipment contains
the following components:
 
     The SLT (System Load Tape),
     The FOS (Fundamental Operating Software) Tape,
     The SUBSYS Tape (containing your optional products), and
     The Powerpatch tape.
 
When a release first ships, it typically doesn't have a Powerpatch
tape, since no patches have accumulated yet.  An Express release is
a retrofit of the SUBSYS tape plus the creation of a new Powerpatch.
The Powerpatch tape can be rev'd on its own.
 
If you were to order Release 5.0 today, you would receive the following:
 
    C.50.00 SLT Tape (first shipped 2/17/95)
    C.50.00 FOS Tape (first shipped 2/17/95)
    C.50.03 SUBSYS Tape (first shipped 11/22/95)
    C.50.06 PowerPatch Tape (first shipped 6/25/96)
 
This combination is considered our "latest and greatest".  Hope that
