Did you do a SHOWDEV 2809? Is it spooled? I don't remember seeing the information. Documentation says:
Files which include a forms message, and which are directed to an
unspooled printer, always trigger a forms message dialog with the printer's operator.
If I remember correctly, replies can appear if FORMID or FORMS (formsmsg) changes from one spoolfile to the next.

No one else at our two dozen locations with 800 + printers have this problem with UT700 however.
