Yes, U.S. Reservists and National Guard are required to sign a form
stating they have someone to take care of their children while they
mobilized.  This is something that is maintained by their Command, (in
an area somewhat similar what we civilians call an HR dept.)  

If their form isn't maintained, of course the Reservist has to hustle to
get it done.  If isn't taken care of it is the Reservist who gets in
trouble (It is a "should have known better," or "where were YOU during
indoctrination?" kind of problem.)
