I have to admit, at first I didn't believe you. I have never run REFORMAT before.
But I found the documentation, and boy is it old

So, I took a peak at the program... from what I can tell, and this is a guess, try
FILE REFFILE= (For REFENTITY)
FILE OUTFILE= (For OUTENTITY)
Thats a guess from doing a FCOPY ;HEX;CHAR

But, “REFENTITY” and “OUTENTITY” have 9 characters, and should be 8 or less.
