I wrote a utility, called VLIST, which was included as part of a product
called SUPERV, which may be what you're looking for.
Here is a sample run, which produces a FORMLIST print file containing
detailed specifications of a form file (fast or regular source version):
:vlist.superxl.schipper

VLIST lists forms and associated fkey labels for specified form files
Enter fully qualified form file name ('RETURN' to exit)
vttutil.pub.sys

Outfile print file called FORMLIST is produced.
