The problem might be the media.  If you use a brand new tape after cleaning
do you get the same error.   Other possibility may firmware (hp02 and hp03
have seen a problem) or yes hardware

Or, simply move DDS3 to System Core I/O SCSI Bus (free):

N-Class path 0/0/1/0
