Cave diving isn't the ONLY cold water diving that's done either.<

Ice diving is most certainly NOT the most extreme cold water diving. Ice
dives are usually short dives and therefore not extreme in any way. Except
that someone has to drill the hole. 

The fact that there is ice on top is irrelevant. The only thing that counts
is the duration of the dives. Which is obviously true for any water
temperature.
