Someplace I have a paper on this.

My previous employer was a MANMAN shop that switch to
Oracle Apps. We used Formation to create forms for our
Invoices, PO, and other documents, first to print on HP2680A,
then onto laserjets.

We designed the new forms on a PC, and printed them to a FILE
using a LaserJet III driver. Then with some editing (bit hacking?)
you can add the PCL codes for "create a macro", and "execute macro
on a new page".

Then when you print from a ux box, send the PCL-formfile prepended
to your data.
