A bit over an hour ago John Burke comes to grief
with PATCHIX; even after doing everything right:

Yup: Been two or more years ago now, so don't recall off the
top exactly what subsystem was involved (it wasn't TurboIMAGE),
but I went through essentially the same saga as John; with the
(fortunately for me) exception that when I finished the system
did not ABORT because the missing pieces were not critical to
system operation like NL - SL- XL; what I needed just "wasn't
there" (it might have been DICTionary or INFORM or etc.).
Basically PATCHIX just "left something out" in the staging area
(easily confirmable by diving down far enough into the HFS for
the Stage).
Again IIRC at this much later date, the fix involved getting a
new patch and / or a new version of PATCHIX... I didn't get any
errors either; it was a "silent" failure.
So it goes...
