Since so many are not on the list this week, I'll add:

I've come to the conclusion that there is no such thing as economy-of-scale.
It goes back to the same argument made in the book "The Mythical Man Month".
As a project/organization/whatever gets larger and larger, the energy to
maintain the system saps the benefits one would expect to get.  I believe
that what people really mean is "economy-of-volume", where the fixed costs
are spread over more sales.
