The summer 1 a.m. and summer 1 p.m. blank student rating of faculty
packages will be distributed today.  During the summer, only
instructor-course combinations not rated during the fall or spring
semesters will be evaluated.  IF you get an evaluation package for a class
that was evaluated in the fall or spring, please return the package to me
with a note indicating it was evaluated during one of those
terms.  Completed evaluation packages must be returned at the end of the 1
am/pm terms.  Please, if you did not receive an evaluation package and
would like your class evaluated, contact me by phone (4288) or by e-mail,
and I will be happy to prepare a package.

Also, please note a couple changes to the memo that is being distributed
along with the evaluations.   The offices of Planning, Evaluation &
Institutional Research and Testing have moved to Sisken Portable Building
B, and that is where completed evaluation packages should be returned, room
102.  IF the class being evaluated is an on-campus night class and cannot
be delivered to my office between the hours of 8am-5pm, please return the
package to the Department of Public Safety (campus police) on Palmetto Street.
