I have received a couple of Faculty Committee Preference forms without a 
name or signature.  Please be sure to fill in at least one of these blanks 
or I will be unable to determine your preference in committee assignment 
for next year.  You could be assigned to a committee you would not like or, 
worse still, be left off a committee altogether.
