It has been over a decade since I have done anything on HP3000.  However, if I
recall correctly, this can happen if a "critical" process is waiting for IO and
for some reason the IO request can not be satisfied.  And, I think there is a
command ABORTIO to cancel the IO request.  I do not recall the syntax but assume
that it would be ABORTIO pin.
