Each group has their own "rotation list" for the week (or month, depending
on their internal preference). They know who is supposed to respond. If the
primary person for the week does not respond within a specified time period,
the operator on duty, will start calling down the list.

If another member of the group receiving the message knows about the problem
(ie: they may have caused the problem earlier in the day), then they will
contact the primary support person, and either notify them, or take over the
call - since they are the one most familiar with the problem.
