I think you're right about patch/ix version.

Did you restore patch/ix from the powerpatch tape after completing your
autoinst to 6.0 but prior to applying pp2?

If not, that could explain it.

Furthermore,  you were probably NOT presented with the option of "including
all FOS patches", since I believe the patch/ix version contained in 6.0 fos
does not have that feature.
