Go to http://www.networksolutions.com/makechanges/forms.html .  You
will find all the forms here.  I assume you want to change your primary
and secondary DNS servers.  The specific form you want is called
"Service Agreement" and if you follow the interactive prompts from
http://www.networksolutions.com/cgi-bin/makechanges/domain.mod it will
build the form for you.
