Research Survey
As a graduate student enrolled in the Library Science program at Southern Connecticut State University, I am required to conduct a research project and to write a paper presenting my findings. By completing this survey, you are helping me meet that requirement. This survey is straightforward and not intended to make you uncomfortable. If you decide at any time while completing the survey to withdraw, you may stop and your survey will be discarded.
The survey consists of four sections. Part A is to establish the reading environment of your childhood home. Parts B, C, and D are inventories of your reading habits during different stages of your life. To complete the survey, place an X on the lines prior to your answer(s) to each question or circle the letter of your answer.
Your responses are anonymous and will be kept confidential. All surveys will be securely locked in a file cabinet and then shredded before being discarded. By answering the questions below, you are giving consent to participate in this research project.
Please return the survey electronically to [log in to unmask] or by mail to 66 Glenbrook Road, Kensington, CT 06037. Thank you for your participation in this project.
Sincerely,
Carolyn Rae McElravy
