HEALTH INSURANCE:  John Deere HMO & POS Participants
Effective immediately, participants enrolled in either the John Deere HMO
or POS may have their extended duration prescriptions filled at the retail
pharmacies below (in addition to the home delivery option still available).
Plan members will be able to receive a three-month supply of a wide range
of prescription drugs, with only one co-payment, provided the drug meets
the Plan's guidelines.

Name of company carrying the state Dental Insurance Plan has been changed
from Fortis Benefits Insurance Company to Assurant Employee Benefits - no
change in dental plan benefits, phone numbers, or other contact
information… just the name change.
