I couldn't find as convenient a link for Georgia, but I did find a few leads.

St. Joseph's Hospital in Atlanta has a page describing advance health care
directives.  It doesn't contain a link to forms, but it says you can get them
from the "Medical Association of Georgia, the State Bar Association, the Georgia
Hospital Association or your local hospital."

This site has a living will for Georgia, but I couldn't find a durable health
care power of attorney form:

This next site *sells* both forms.  There are sample forms available in PDF
format that have "SAMPLE" watermarked on them when they print.  You can either
buy the forms (they're rather proud of them), or type the text from the samples
into your own document, or (I guess) try to use the ones with "SAMPLE" on them.
  (Since I am not a lawyer and do not play one on TV, I don't know whether that
would affect their legal validity.)

Not as neat a solution as the single file I found for Tennessee, but I hope it
helps.  If anyone else finds a better link to free forms, please feel free to share.
