I had the same problem. At the top of that screen it asks for your shipping information. Then it asks for the bill to address beneath that, but it gives the option to just say that the bill to is the same as the ship to address. So when you use that option, it leads you to believe that you don't have to fill in the bill to address. But the first item on that list is a drop down menu for the entity code, and you just click on UTC. I went ahead and filled it out anyway because I kept getting message that the form was incomplete. When I filled in everything, it finally took it.

I just attended the Staples training session and when I tried to Register -
after entering all my information and then hit SUBMIT - it came back and
asked to submit an "Entity Code" I don't believe this was covered in the
training session and I do not understand what to do.



Doe anyone know what is meant by an Entity Code?
