To those who are advising students for classes, below is some information
about the Student Health holds.  Please pass this information along to any
student that you find with a hold from Student Health.

All students are required to submit documentation of 2 doses - MMR
(measles, mumps, rubella combined) immunizations to Student Health. NO
ORIGINALS--COPIES ONLY!! If students do not have documentation they may
request their immunization records from their physician, health department,
high school, or last college attended.
The following information is required:
1. Complete Name
2. Date of Birth
3. Social Security Number or Student ID Number
4. Dates for both doses of MMR (1st dose is given around their first or
second birthday, 2nd dose after that)
New students are given one semester to submit their immunization records.
We are allowed to override only 1 time within the 1st semester. All
students returning for a 2nd semester must provide this documentation
before they can register.
Students can send immunization requirements via fax, but must have all the
