I finally located a copy of the SPUMS pre-dive medical form for entry-level
recreational diving. It may be slightly out of date, this one was reprinted
Dec 1996. Its too long to send to the list but if anyone wants a copy I
will send it privately.

SPUMS publishes a list of Docs who are appropriately trained & qualified to
do diving medicals. If these Docs have any problems during the consultation
they can refer their budding wannabe diver to a specialist diving physician
(Dip DHM), who may even refer them on to other specialists for further
tests eg cardiologist, respiratory physician, etc.
