A DM is enough.

No.

For a swim-through, no. For a cave or cavern, the honest answer is I don't
know. I would certainly never dream of taking a student into one (and would
shy away from going into most of them myself anyway). But I am sure there
are DMs and instructors who would, just as there are people who break the
speed limit any time, and people who dive to 30 meters on their first
unsupervised dive after certification.
