Never.  I've come close a few times while kitting up or moving about a boat
not really made for diving, my own for example.  I've pulled some muscles
avoiding a fall, but I've never actually fallen while kitted up.  Even the
idea of falling like that is enough to ensure that I'm very, very careful.
It gives me the willies just to think about it.
