During a period of transition, the Division of Student Development and
the School of Nursing are partnering to provide health services
including immunization reviews for the students of UTC. Students should
fax the immunization form found on www.utc.edu/Orientation site to
423-425-5357. These forms will be reviewed by the School of
Nursing--those individuals who do not have complete records will be
placed on hold. Please remember that the UTC Immunization Checklist must
be completed in its entirety and include a signature.  For more
