Hi Michael,
DBPUT error 95 is not documented in the Image manual but was documented
in a Response Center engineering note that should be at the ITRC although I cannot
seem to pull it up. So... here is the text from that note, I hope this helps

The Image status condtion word 95 means a broken chain was encountered in
the database.
Image status 95 unknown error condition is a broken chain.
One possible cause: Any 9X image status could be caused by a bad restore of the
database. If the store was not complete or had a problem then the restored
database will have broken chains.
How to fix:
If the problem was seen after a restore of the database, create another
store tape. It's possible the orginal store was not complete for some reason.
You can fix the broken chain using a 3rd party product Adager by REGO or
DBGENERAL by Bradmark. If these products are not available then doing an
unload and load of the database can be done. Some data lost may be
experienced.
Other possible reasons for this error:
Broken chain could be caused by some type of hardware problem. The
recommendation would be to fix the broken chain and enable dumping on the
image database. The dump will give us some idea what happen to cause
image to abort and may gives a clue as to what caused the hardware problem.
Once a broken chain has occured, it must be fixed as soon as possible. If
the cause is determine to be related to a disk problem. The disk will need
to be replace and the system will have to be installed.
