Kent,
  Could be Omnidex is trying to open a different dictionary than QUICK is trying to use?  As I recall, the default dictionary name changed from "QSCHEMAC" to "PHD" around 7.29 (?), but Omnidex doesn't know what version of Cognos you're using so goes down the list. If it finds a "QSCHEMAC" or a file equation pointing to one BEFORE it finds the PHD (or it's file equation), it may be trying to open that first and failing for some reason.
