Last month I posted the following warning :

Just a warning that we're currently experiencing some technical problems
with very large KSAM64 files. HP are currently investigating and suspect
the problem is related to tranasaction Manager (XM) not always working
properly on Ksam64 files during a reboot.

The update I can now give you is that HP have found a bug in their
memory manager which affects these very large files and are about to
deliver a beta test patch to us tomorrow.

The bug only reveals itself if you shut down your HP after writing to
these files AND the shutdown doesn't reach the "shut 6" stage. (If, for
example, you reboot your HP with CTRL B + RS then it is possible that
data hasn't been transferred from memory onto disk and thus the KSAM
file is internally corrupt !).

Any users of software which provides 3rd party indices to Image
databases (and the TPI software has had to build the file using KSAM64)
probably ought to contact HP for the patch once we've finished our beta
test.

I don't think this bug affects CM ksam or KSAM/XL.
