Even if you never update or delete, you still need the sorted chain.  There
is no guarantee that the data for an index will be DBPUT in the correct
order that the sort and extended item would provide.

The newest entry will be the last entry in the chain, physically and
logically in the absence of a 'sort'.

I don't know if that implicit chronological order is sufficient for Michael
Anderson's application or not.  If it is not, then what Mike Berkowitz said
is true, the sort would be necessary.
