Did you verify your setup file content ?  If it includes "[esc]&l2A" for
example (Letter paper size) ?

You may try to start with nearly blank setup_file including only [esc]E to
reset printer and the re-add them until you find offending command ?

Remember you need to stop and restart spooler if you do some changes to
npconfig.pub.sys but as far as I know, you don't if you just edit
setup_file...
