Thanks for all of the help on this one. I had several suggestions. It
turns out we are using a 3rd party spooler package (ESPUL). They quickly
responded to my request for docs (there doesn't seem to be any onsite). I
was then able to determine how to get banner pages turned on. I even got
it right the first try ;)
