I highly recommend Fantasia from Jetform.  It's user friendly
and very powerful. There's a PC portion that allows point and click
form design (importing tiff images, defining field fills,
drawing boxes & lines, etc), but the commands available on the HP
side are so complete and easy to use, it now seems tedious to go to
all the trouble of pointing and clicking, so I just use their Larc
editor to do everything.

There's also a package called Formation, now supported by Roc
software.  It also has the PC package.  We were using it a few years
ago, but felt Fantasia more completely fit our needs.
