I had this happen on our 997-400. It turned out to be a REALLY bad disc
drive in one of the Jamaica boxes. Apparently it was so bad/noisy that it
hung up the device polling and the system couldn't see any devices after
that ldev in the configuration. Even Mirror/iX didn't help us that time.

After much checking around and various other attempts, it turned out that
simply pulling that bad drive out let brought the rest of the system back to
normal (it took us a while to determine that it was in fact just a single
drive causing the problem).

Subject: [HP3000-L] after adding the disk ----
???????

                Hi All,

Today I fixed 4X4 GB (16gb) with a Jamaica box to my system.
I added
successfully. I added the path/added the volume. During
Dstat it shows me as
a normal disk. even first disk started the synchronisation
the data. It was
92%. It seems to me now every thing is working fine.

But when I brought up the system my nbspool job was not
running. I checked
the $stdlist found device is not mounted. Then I checked
showdev I was able
to see only 22 ldevs. in place on > 250. Also I was not able
to logon to the
system through telnet. Even all the services was up. ???

Luckily b4 adding the disk I prepared the CSLT. Now I am
recovering the
system through CSLT tape. Data is still under restoration.

Why this happen. With the same procedure in early morning I
installed 8X9gb
disc with the Jamaica box. That system is working fine. But
what happened to
                this system.???????????/
