Yes, server.apache must have MPE permission to read the file and (I think)
"read and execute" (posix terminology) the directory containing the file.

I'd suspect the directories in between htdocs and your file. It would appear
that your file has sufficient permissions to be read by anyone, so that is
why I'm thinking it is the directories.

Sorry, can't help here, althought I eventually got Apache serving a cgi
script to run Quiz at my old employer.
