I've seen similar errors when a system/boot file had extents on other
than ldev 1, but in those cases the files loaded OK after the directory
entry was purged.  Looks like yours failed twice, haven't seen that.  I
looked ahead and saw Stan's post that it was "Directory Services" error
and that doesn't ring any bells, unless your INSTALL.SYS is outside of
ldev 1.

Can't offer you any options if the update failed and you can't boot.
