First, from a hardware viewpoint, ensure that drive is properly 
installed in jamaica box and has a distinctive SCSI address. Should 
confirm with ODE MAPPER offline diagnostic.

Then, from MANAGER.SYS, use sysgen to add disc drive logical device 
number with appropriate path and id. For example,

:sysgen
io
ap 10/4/12.4 id=pseudo
ad 99 path=10/4/12.4.0 id=hvddisk (assuming ldev 99, path is correct 
path, and drive is fwscsi drive)
ho
exit
ke
..
..

Then reboot with "start norecovery"

After reboot,

:volutil
newvol mpexl_system_volume_set:member?? 99 100 100 (if member of 
system volume set - otherwise, replace long_name with appropriate 
volset name - substitute ?? appropriately)
exit

Voila!

You could add the drive dynamically via IOCONFIG - but I would not 
advise doing so.
