Well I got an answer about this:

1) Ensure that ALL the volums in the system volume set have at least ~2048
contiguous sectors.
(thanks to everyone who pointed out #1)

2) Make sure that you've done a :strcntl.net.sys -S after you're system
starts up.  (I put it in the startup UDC)
(thanks to Cathleen at the HPRC!)

Once I got some disk space, I still had the problem.  Doing the
STRCNTL.NET.SYS -S didn't help until I rebooted.
