Mark,

I was going to suggest the same thing thinking there must be a config
problem at a higher level than the drive.  The reason I didn't is that the
drive was found by mapper.  To me, that almost eliminated the possibility
that it was a configuration problem.  Do you think this could be a signal
quality problem between the device and controller?
