You should not need to either format nor scratch volumes that are in the
UNKNOWN state.

If you did try to format and it began but was then interupted for whatever
reason, you will need to re-format.

Otherwise, if it completed alright, the fact that the newvol command is
failing suggests a physical problem with the drive.

Rebooting may help, but I doubt it.
