The New York Times had a story about the airport in Sweden.
They couldn't issue emergency temporary passports to their citizens
because their software treated 99 in a special way, so some people
had to miss flights.  They fixed the software before the day was
over, though.

I have a client who couldn't close the books because a program
added one to a two-digit year (from the system date)
to calculate some cutoff date.

I'm sure there are more stories, but probably not many that have a
big enough impact to get into the news.
