Do a showdev on the device, is it "SPOOLED     SPOOLER OUT"?

The other common spooled device REPLY, that I recall is when using a FORMID on the FILE equation.  It asks you to mount the special paper.  

FORMID              Applies only to output spoolfiles.  A string of up
                    to 8 alphanumeric characters, beginning with a
                    letter, which uniquely identifies a special form
                    which is to be mounted.  A message displaying this
                    FORMID is printed on the system console or $STDLIST
                    of the associated user of the spooled device.  The
                    spooler process then awaits verification that the
                    special forms have been mounted before printing the
                    file for which the FORMID was specified.  The
                    default is that no FORMID or message is displayed.

The only time I ever replied to the console is to tell it the device number of a tape drive.

The behavior Tracy is describing is NOT limited to Fantasia forms files.
Yesterday:
We are seeing 'reply requests' on the console for simple offline listings from Editor
- I issue a file equation:   FILE EDTLIST;DEV=2839
- I start EDITOR, text in a file and issue LIST 1/30, offline
- a reply appears on the console and nothing prints until the replay is answered

Overnight we adjusted the 'sleep' settings on the printer (trying to get sleep turned off).

This morning:
The printer is working 'as expected' (i.e. no replies being requested)  I'm not saying that changing the 'sleep' settings on the printer caused the changed behavior, but it is the only think I know of that changed.
I don't understand what the sleep settings on the printer would have to do with a reply being required.  (but I also don't know the deep dark interworkings of the spooler either)

1) has anyone witnessed similar behavior?
2) exactly what causes the 'reply' requests?

We're getting the oddball console reply for a spool file on an LDEV for Fantasia forms files.

I've looked at NPCONFIG, SYSGEN>IO, and LARC's LINEPARM and FSPCONF and for the LDEV and see nothing out of the ordinary.

Unless there is something in the Environment file were using.
