I had a similar problem when upgrading to 6.0.

When one of the installation jobs tried to create an account, it failed with
a Duplicate Name in Directory error.

The account did not exist, you could not purge it but it could not be
created either. It turned out that a the user entry being specified for the
account manager had somehow been created for the account, even though the
account did not exist.

To fix it, I created the account with another manager user name and was then
able to delete the existing user. I then was able to purge the account and
let the installation job create it as needed.

I have been working with the HP3000 for over 10 years and love the stability
of the platform. However, the platform has become increasingly more fragile
since they added posix to MPE. I have had more system crashes and other
problems in the last 2 years, than in the previous 8 combined.
