We use a slighlty different address format at Robelle.  Internally,
we store the address as
 
   Company Name    x60
   Contact Name    x40
   Contact Title   x40
   E-mail address  x80
   Street-Lines   3x46
   City            x46
   State/Province  x46
   State/Prov Code  x2
   Country         x40
   Zip/Postal      x14
   Phone           x22
   Fax             x22
 
Why 46 characters? We found 40 insufficient. Sometimes you try and cram
the following (most likely Government or Boeing) address all on one
line: 15399-102A Avenue Suite 1460 Building 12
 
Note also that phone and fax are 22 characters, not 20. This leaves room
for a 6-digit extension, and YES some phone systems do have that many,
i.e. (604) 582-1700 x123456.
 
Canada Post issued a brochure awhile ago stating that the address should
be formatted in the least important to the most important information
(top to bottom). The address should be in uppercase, as well, so that
their machines can more easily read them. There should be no punctuation
in the state/province code, postal code, or country name. For example,
